Officers and Staff (Teaching or Non-Teaching personnel from the Regional Office
and from the different divisions
Requirements:
Filing of Complaint

Full name and address of the complainant
·
Full name and address of the person/s complained
of, as well as his/her/their position/s and office in the DepEd-ARMM
·
A narration of the relevant and material facts
which show the acts or omissions as allegedly committed by the person/s
·
Certified true copies of documentary evidence and
affidavits of his/her witnesses, if any
·
Certificate of Non-Forum Shopping
·
Complaint shall be under oath
Filing of Appeal
·
Notice of appeal
·
Appeal Memorandum (3 copies)
·
Proof of payment of the appeal fee
Filing of Motion for Reconsideration
