Requirements:

Pre-School and Elementary
·         Birth Certificate for pre-school and Grade I pupils
·         Report Card for Grade II-VI pupils/students

Secondary
·         Birth Certificate for first year students
·         Report Card
·         Certificate of Good Moral Character
·         2 ID pictures for first year

Transferees
·         Certificate of Good Moral Character
·         Forms 137 and 138
·         Reading ability assessment / Placement Test


Schedule of Availability of Service:

      Pre-School and Elementary: Two (2) weeks before the start of the academic school year
     Secondary: April-May
